Skilled Nursing & Short Term Rehabilitation
Our two skilled nursing home communities serve both rehabilitative and long-term care residents with individualized care plans. They are conveniently located in Mondovi and Menomonie.

Autumn Village in Menomonie is a residential care facility blending senior independent living apartments with supportive services. We offer residents security, privacy, social opportunities, and more. The building is attached to ALH Menomonie on the edge of Wakanda Park.

Galloway Homes is a 32 unit subsidized apartment building for seniors and disabled residents. It is located along the beautiful Galloway Creek that winds through the south side of Menomonie.
