Autumn Village Assisted Living community is designed for those who need assistance with personal care, socialization, nutrition as well as assistance with medication management, ambulation, or housekeeping chores. We are located next to American Lutheran Homes and beautiful Wakanda Park.

Autumn Village provides a comfortable, individualized living space furnished the way you want it. Our apartments are spacious with a private bath and kitchen to help maintain the private living you prefer. The dining area, cozy lounges and courtyard with a walking path, provide plenty of common space for you to meet other people from your “neighborhood.”

         A neighborhood is exactly the feel Autumn Village strives to maintain.

We promote independence and camaraderie through outings, activities, social gatherings and intergenerational events. Enjoy socializing with your neighbors as you attend the following:

We also recognize and respect the need for individual private time. Residents, family, and staff work together to assure assistance in maintaining a maximum level of independence tailored to each individual.

Autumn Village provides senior care services as needed for those who benefit from personal care assistance but do not require the full professional services of a registered nurse. Health monitoring services provide you the opportunity to choose your own form of health care

 Our staff is available 24/7 to meet your needs. A registered nurse coordinates health monitoring, support and referral services for each resident.

 The Universal Worker concept is applied to each and every staff member at Autumn Village. This phrase simply means that all staff are competent and capable of performing whatever tasks you require. You will benefit from the continuity that comes from staff who are familiar with your individual service plan.

Personal care workers are available 24/7. A program director oversees the well-being of the entire Autumn Village community. Should additional needs arise, professional staff are also available for one-on-one consulting.
